Improving the focus on population health and prevention in primary health care is a key activity of Primary Health Networks (PHNs) and related organisations. 

With that in mind the Preventive Health Matters web tool has been purposely developed to facilitate knowledge transfer and to harness the use of research and evidence to improve the planning, implementation and evaluation of population and preventive health initiatives. 

This web tool will support PHNs and related organisations across Australia to deliver an evidence based focus on population health and prevention; sharing ideas, resources, and contemporary evidence in a collaborative way. 

The Northern Territory PHN is supported by funding from the Australian Government under the Primary Health Network Flexible Fund to develop and facilitate the Preventive Health Matters web tool.
